About the Author
Patsy Danehy Catsos is a registered dietitian in private practice; her special area of interest is gastrointestinal health. Her trailblazing book, IBS--Free at Last! (Pond Cove Press, 2009), introduced U.S. health care providers and consumers to FODMAP-restricted diet therapy for irritable bowel syndrome (IBS). Patsy is the editor of the blog IBSfree.net, and an expert contributor to Sharecare.com, an interactive social Q/A platform created by Jeff Arnold and Dr. Mehmet Oz in partnership with Harpo Studios, HSW International, Sony Pictures Television, and Discovery Communications. Ms. Catsos earned a B.S. in Nutritional Science from Cornell University and an M.S. in Nutrition at Boston University. She completed her internship at Boston's Beth Israel Hospital. 50 years of pain and swelling is GONE

The FODMAP elimination diet, and this book, saved my life. Chronic swelling and constipation and pain have followed me for 50 years. Going strict gluten-free for 3 years only made it worse in the end. I had NO IDEA what was causing my gut such distress. My doctor (an internist, not a GI doctor) suggested looking into FODMAPs. OMG. The second day I was on the FODMAP elimination diet, at 4:30 in the afternoon--by which time my gut had ALWAYS swelled up, I looked down at my still-flat abdomen and said out loud, "WHOSE BODY IS THIS???!!!" I am so, so grateful; there are no words.

Tip: since FODMAPs are cumulative in the gut, it make take you a bit of practice to find out what is an okay cumulative amount in a day, and what is not. When you find you're having symptoms, you overdid it. Reduce your total FODMAP intake the next day. I found out that I was overdoing even the allowed fruits; I went without fruit for a couple of days, and had zero pain. Then I proceeded with caution with the fruit.

If these fermentable sugars and fibers are your problem, you're on the road to Glory Land! Good luck!

Comparison to other diets for IBS

For the last two years, I have been on a quest to find the right diet for myself that will accommodate all my food restrictions, not worsen my digestive symptoms and actually will allow me to maintain or lose those creeping middle aged pounds. A tall order for any diet.

Going gluten and dairy free may have actually unmasked or worsened my IBS symptoms! The Paleo diet (greens and meat) fit my food restrictions and made me feel energetic (I lost weight for a few weeks). Unfortunately, a gradual worsening of IBS symptoms led to a desperate state after about 6 months. Then I learned about soluble vs. insoluble fiber impact on IBS and I brought back all the soluble fiber that Paleo had eliminated (potatoes, rice, beans, etc. ). My IBS symptoms were mostly relieved (Whew!) but I miss the light and energetic feeling I had on Paleo. The Blood Type Diet makes sense to me because as a blood type A, I should be mostly vegetarian, which fits with how I used to eat before all this IBS started. However, the Blood type diet is more difficult due to so many specific "no-nos" - like tomatoes and peppers! What's life without a little spice?!

My IBS still seems to come and go and I'm not sure why. So now I found Patsy Catsos book IBS-Free at Last! 2nd Edition on Kindle. This book is not about food restriction or rigid philosophies. It is a road map to personalize your own diet. If you have certain food philosophies you ascribe to, you can incorporate them into this diet. Once you learn which types of carbohydrates are problematic for you personally, you can avoid what you want or choose to challenge your system and bear the consequences. At least you'll have a good idea what caused any problems so you can make better choices for yourself.

The book provides an education on why high fiber or soluble fiber diets do not solve the IBS dilemma for many people. However, I found that consuming excessive insoluble fiber from green vegetables and many fruits did worsen my IBS, so maintaining the soluble fiber balance was helpful to me. Ms. Catsos' 2-week Elimination Phase includes a lot of soluble fiber foods, so it does not actually contradict the alternative approach to IBS dieting. It is no surprise to me that people feel better immediately at the Elimination Phase, as it is a basic, healthy, whole food diet. It's hard to find prepared foods that meet the criteria, so you may need to do more cooking on this diet. Vegans will be challenged to find protein at the Elimination phase, but nuts and tofu are okay. This diet does not guide you on quantities of any food group so you need to determine quotas for yourself (protein vs. carb vs. fats). The challenge phase is faster than other diets but very thorough - you add more than one food at a time but they must all be in the same carb group. You can choose the order of the groups but within each group there is a prescribed list (not required to eat everything on the list).

I like using the Kindle version but there are a couple of disadvantages. It is hard to make a paper or electronic copy of the symptoms lists and FODMAP foods lists off the Kindle. Patsy Catsos has a website which can provide you some of the lists in Acrobat reader format. Also, it is challenging in the Kindle format to use this book as a reference book, once you have done a read through. I find it more inconvenient to skip around on the Kindle version than an on-paper version. I wish there were an interactive FODMAP list in a PC version (such as Excel) or a smart phone app for the FODMAP to help track your diet and symptom progress. That is my suggestion for next steps for Ms. Catsos!

Truly Free from IBS

This is an update to my original review:
September 2012
It has been 4 months since I received my copy of IBS: Free at Last! and what a difference the 4 months has made! I thank Patsy Castsos for providing the framework that allowed me to succeed in being FREE from IBS pain and now FREE from IBS medications! And if being pain free wasn't a great enough gift, I have also been able to lose 50 lbs in just 4 months. I do understand that weight loss is NOT the intention for this book, especially since many IBS sufferers are underweight and cannot afford to lose weight. But in my particular case, eliminating high FODMAP vegetables has allowed me to enjoy fresh vegetables and finally succeed in eating a well rounded diet that allowed me to lose weight. I do believe that 20-30 lbs that I lost were connected to the bloat and water retention of IBS and being that I am IBS free, those pounds are staying off! The time it takes to follow the elimination phase of this program is time worth spent. I know it seems daunting and impossible to eliminate things like garlic, onions, wheat and even broccoli. But, I have and I am so glad I did. It turns out that for me, these 4 ingredients were the main part of my previous diet. What was a salad without red onions? How would I cook without garlic? I have learned to have a varied diet, full of nutrients, without being sick. Now my salads are full of a variety of low FODMAP vegetables and I can actually eat as much salad as I want ... a huge change for me!
On the advice of my doctor, I still need to lose another 50 lbs, but knowing I can eat well without suffering pain, bloat, gas and many trips to the bathroom, I know I can continue to lose weight and exercise. I believe that my doctor and Patsy Castsos gave me the best gift in the world ... my healthier self!

Thanks to my GI doctor, I went searching Amazon for a book about low FODMAP diet. Well, I found the BEST book written by an understanding and compassionate author. Patsy Castsos is clear, straightforward and supportive in her book about how a low FODMAP diet can change your life! And it has changed mine. I have already purchased two copies of this book as well as recommending it to my GI doctor, who bought the ebook, and my nutritionist. I felt better by day four, and I am symptom free for 6 weeks plus lost the 20 lbs of bloat/water my body has held onto for over 20 years!!!
